This could be the perfect position for you !
Hi my name is Rupali Vyas and I am a Board-Certified Neurologic Clinical Specialist and founder of Congruence Physical Therapy & Wellness located in Sunnyvale, CA (Bay Area).
Congruence is a growing, highly specialized neurologic outpatient physical therapy practice seeking a Physical Therapist who is fascinated by neuroplasticity and treats the human nervous system like the ultimate puzzle . The role begins part-time with an immediate path to full-time as the clinic continues to expand.
But before I start asking you questions, I’d love to share a bit about our practice and clinical philosophy
-
The "Zone of Discomfort": While patients practice safe, comfortable movements at home, the clinic is designed to safely push them into their "zone of discomfort." Interventions are uniquely designed to challenge balance limits, improve movement confidence, and reduce fear-avoidance behaviors.
-
Addressing the Mind-Body Connection: Our practice heavily incorporates principles of neuroplasticity while actively addressing the anxiety, stress, and secondary emotional barriers that accompany chronic dizziness and neurodegenerative conditions.
-
Hybrid Model: The clinic operates as a hybrid practice, accepting Medicare alongside a high-value, complete cash-pay model.
-
Uncompromising 1:1 Care: Every session is a dedicated 45 to 60 minutes of uninterrupted, individual care.
-
Value-Driven Outcomes: Ranging from acute, optimistic resolutions (like clearing BPPV) to building long-term, consistent exercise habits for chronic neuro patients.
-
Balanced Documentation: Standard Medicare documentation and billing are required for each session, but the ultimate focus remains entirely on maximizing the quality of care.
Core job expectations-
The Perfect Blend of Art and Science
We are looking for a clinician who is equal parts clinical expertise and a compassionate communicator.
-
The Science (Hard Skills): You know your way around standardized neurologic evaluation, canalith repositioning, and manual handling for all mobility levels. You feel comfortable using tools like infrared goggles and safety harnesses to nudge patients into their "zone of discomfort."
-
The Art (Soft Skills): You don't just treat symptoms; you listen to the person. You excel at understanding the complex emotional layers of neuro conditions, practicing with deep compassion and treating therapy as a collaborative team effort.
Ultimately, you are someone who knows how to deliver and communicate massive value, transforming a first-time patient into a deeply connected, long-term client.
Impactful & Creative Clinical Execution
-
Passion-Driven Care: The core expectation is a genuine passion for neurologic PT and a desire to make a measurable, life-changing impact on patients' lives.
-
Evidence-Based Creativity: Clinicians are encouraged to think outside the box and design innovative interventions while remaining anchored to the best current clinical evidence.
True Professional Autonomy
-
Independence & Flexibility: While mentorship and guidance are readily provided, the daily routine is highly autonomous.
-
Scheduling Freedom: The therapist has the independence to structure their own schedule, decide which patients to see on specific days, and pace their day effectively.
-
Clinical Exploration: Freedom is granted to learn new techniques, test fresh clinical approaches, and continually expand your professional skillset.
Multi-Directional Feedback Loops
-
Constructive Performance Feedback: The ideal candidate is receptive to professional feedback regarding their clinical performance and effectiveness.
-
Client-Driven Adaptation: Because the practice treats patients as autonomous partners in their own care, the therapist must be open to direct feedback from clients and agile enough to adapt treatment plans to meet the patient’s evolving needs and desired outcomes.
Ideal Candidate Profile: Desired Characteristics & Traits
1. Clinical Demographic & Special Populations
Lifespan Versatility : While the clinic primarily serves proactive seniors (65+), there is a healthy mix of middle-aged and young adults. Note: This role does not include a pediatric population.
Cognitive & Chronic Complexity: The ideal candidate must be equipped and eager to work with complex cases, including patients presenting with cognitive deficits and chronic symptoms (e.g, chronic dizziness or balance issues).
2. Core Personality Quirks & Mindset
Radical Patience & Calm Demeanor: Complex neurological cases demand a clinician who remains calm under pressure, acts as a stabilizing presence, and patiently navigates fluctuating mobility levels.
Strengths-Based Approach: Rather than focusing on a patient's limitations or what they cannot do, you naturally default to identifying what they can do, using that as a foundation to build their capacity.
Curious, Creative, and Adaptable: You possess an innate curiosity to solve complex puzzles and the creativity to modify interventions on the fly—especially when working through cognitive barriers to achieve successful functional outcomes.
3. The "Clinician as a Coach" Philosophy
The Coach vs. The Paperwork Provider: We are not looking for someone who simply writes a Home Exercise Program (HEP) and hands it off. You must view yourself as a dynamic coach and guide—someone who actively teaches, works with the patient, and builds their independent movement confidence.
High-Energy Group Leader: You have the charisma and enthusiasm required to safely guide and motivate Parkinson's disease specific group classes through high-intensity exercise protocols.
Respect for Senior Drive: You harbor a deep appreciation for the senior population, recognizing that many older adults are highly motivated, lead active lifestyles, and possess an inspiring drive to improve regardless of age.
4. Interprofessional Collaboration
Confident Communicator: You are comfortable communicating and collaborating directly with a patient's broader medical team—including neurologists, psychologists, and psychiatrists—to advocate for, adjust, or modify interventions based on the patient's comprehensive presentation.
We currently have a lot of flexibility in the schedule and could bring on someone needing just a few half-days, and then work all the way up to full time needs.
If all the above has made you excited about this job, here is a detailed "success description" outlining all the details of what being successful in this position will look like:
To achieve "Superstar Status" in this position, you will:
1. Elevate the Culture & Environment
Bring the Energy: Smile, laugh, and make others do the same—ensuring patients feel exceptionally welcome and cared for every day.
Support the Team: Value your co-workers, care about their lives, and actively show gratitude to the administrative staff.
Protect the Vibe: Never take our unique, positive clinic atmosphere for granted compared to standard outpatient grinds.
2. Adopt a "Lifelong Guide" Clinical Mindset
Be the Guide: Look past the dysfunction; we are a lifelong primary provider helping clients stay active and healthy.
Treat the Goal, Not the deficit: Focus on what the patient loves to do that their impairment is currently preventing.
Walk the Walk: Be a physical representation of health, fitness, and wellness yourself.
3. Master Ethical Sales & Practice Growth
Embrace Ethical Selling: Understand that "sales" is how we successfully get people the help they need to change their lives.
Convert Prospects: Learn and apply strategies for free consultations to maximize new client onboarding.
4. Build Community & Authority
Amplify Success: Compel at least one patient per month to leave an online review of their experience.
Innovate Proactively: Constantly think of big-picture ideas to improve business operations and patient care.
5. Keep a Flexible, Growth-Oriented Focus
Commit to Mastery: Remain open and excited to learn new clinical and experiential strategies to help patients hit 100% of their goals.
